Lesotho's current flag was adopted on January 20, 1987 after a military coup in which the military replaced the governing Basuto National Party. That's why the shield and spears are on there; they signify defense.

It was a dark blue flag with a white conical hat from the national folklore. On the hoist there were two thin vertical stripes of green and red. W. Smith explains the colours as: blue - sky and rain, white - peace, green - land and red - faith. It was officially hoisted on October 4, 1966.
The Royal flag was similar, having a bit smaller hat above the states coats of arms.
The coat of arms represented in Smith's book has a shield that is much like the device on the new Lesotho flag. I don't know if it was changed together with the flag.
